Program Copyright Agreement
Use of teaching material—the reproduction or utilization of the material provided, whether in part or in whole in the Pilates Master Mentor Program is copyrighted material and the use in any form, or by any electronic, mechanical or other means now known or hereafter invented, including xerography, photocopying and recording is hereby prohibited without the written consent of Lolita San Miguel.
Any citation or quote of any statement or statements made by Lolita San Miguel used for commercial purposes must have the previous written consent of Lolita San Miguel.
Ms. San Miguel does not endorse any commercial product of any nature and cannot be quoted as having endorsed any product of any nature.
Any recording made, whether photography, videography, cinematography, audio or by any other means during the Pilates Master Mentor Program (PMMP) is considered teaching material and is governed by Clause # 1.
Pilates Master Mentor Program participants are prohibited from making any statements to the effect that they are Second Generation Pilates Teachers, Lolita’s Disciples or Lolita San Miguel Pilates Masters until they have completed the Pilates Master Mentor Program course successfully and granted the official Diploma signed by Lolita San Miguel.
Code of Ethics (itemized below)—Ms. San Miguel expects all of her Disciples to adhere to the highest standard of professional ethical behavior as a Pilates Teacher. The proof that a Disciple has violated of any of these ethical standards or any of the clauses herein mentioned may result in their removal from the list of Disciples in Good Standing and may receive a negative recommendation from Ms. San Miguel.
Continuing Education—Ms. San Miguel expects all Disciples to continue their education as per the conditions established by the Pilates Method Alliance, of which Ms. San Miguel was a co-founder and a member.
Disciples who elect not to use the teacher training program developed by Ms. San Miguel, Lolita’s Legacy TM and use their own or another Pilates teacher teaching program may not say that they teach the Legacy of Lolita San Miguel.
Lolita San Miguel methodology, since the Mentor Program is designed for the professional development of established Pilates professionals.
Pilates Master Mentor Program
Waiver of Liability and Informed Consent
I,______________________________________ (Please print your name), hereby agree to the following:
1). That I am voluntarily participating in the Pilates Master Mentor Program (hereinafter called “The Program”) offered by Ms. San Miguel, during which I will receive information and instruction. I recognize that this fitness program involves physical contact and requires physical exertion which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.
2). I understand that it is my responsibility to consult with a physician prior to and regarding my participation in The Program. I represent and warrant that I am physically fit and I have no medical condition which would prevent my full participation. I agree to assume full responsibility for any risks, injuries or damages, known or unknown which I might incur as a result of participating in The Program.
3). In further consideration of being permitted to participate in The Program I voluntarily and expressly agree to hold harmless Ms. San Miguel and The Program, their officers, agents, instructors or employees from and against any and all claims, loss, injury, damage, cost, charges and expenses, including attorney’s fees, which the undersigned may suffer, such as any injury or death caused by their negligence or other acts, as a result of participating in The Program.
4). The undersigned agrees that no refunds will be given during the course and acknowledges full responsibility for payment in its totality once enrolled.
5). I understand that until I complete all of the hours of instruction I will not be eligible to receive the Pilates Master Diploma signed by Ms. San Miguel, nor do I have any right to use the term “Pilates Master” unless I have been awarded the Diploma signed by Ms. San Miguel.
6). The course requires that you pass with a 75 + score on each of the sessions. Make up or retake sessions are available at a 50% of the cost.
I have read the above enrollment form and waiver of liability and fully understand its contents as well as the Refund/Cancellation Policies. I voluntarily agree to the terms and conditions stated above.
